Clinical Care Manager - Aged Care

The Opportunity

We are seeking an experienced Clinical Care Manager who is ready to take their next step in their career. You'll be joining a leading not for profit Aged Care provider who is well established and proud of their commitment to the community. In this business-critical position you will bring strong clinical experience, be well versed with leading and driving clinical services to ensure positive outcomes are achieved for our clients' residents.

Key Responsibilities

• Collaborate with the Facility Manager to oversee continuous improvement, OHS, and training programs, ensuring care standards are met.
• Work with medical and allied health professionals to support positive care outcomes.
• Provide recommendations and coordinate referrals to specialists following clinical assessments.
• Monitor AN-ACC reviews and ensure accurate documentation to optimize consumer care funding.
• Managing of a large memory support unit
• Confidently lead and support staff effectively in a large facility

Qualifications and Requirements

• Current unrestricted RN AHPRA registration
• Bachelor of Nursing or equivalent qualification
• 2+ years of senior clinical nursing experience within Residential Aged Care
• Confidence and driven to nurture team development and supportive culture
• Knowledge of Aged Care Quality Standards, Code of Conduct and Charter of Resident Rights
• Exercise initiative relating to organisational policies and procedures
• Satisfactory national police check
• Valid Australian working rights
